I don't know anything about the movie, but the eyes clearly seem important based on the poster. Two eyes missing, two eyes, and then you see what looks like optic nerves. "Unseen" world of terror. Don't know what it all means. But my guess is you lose your eyes and you've got to run for your life or he will have to cut out his eyes as a mechanism to survive the terror.
